2.	Last date of sale of forms by post :	Monday, 16th January, 2012
3.	Last date of receipt of completed application form :	Tuesday, 24th January, 2012
4.	Dispatch of Admit Card :	Friday, 10th February, 2012
5.	Date of Entrance Examination :	Sunday, 4th March, 2012
6.	Display of Key on Website :	Wednesday, 7th March, 2012
7.	Last date to entertain any complaint regarding Key:	Wednesday, 21st March, 2012
8.	Date of Result on website / IMS Notice Board :	Saturday, 31st March, 2012
9.	Date of First Counselling :	23rd & 24th April, 2012
10.	Date of Second Counselling :	14th May, 2012
11.	Date of Third Counselling :	23rd May, 2012
12.	Date of Fourth Counselling :	30th May, 2012

Application form can be obtained from Bank of Baroda, BHU branch, S.S. Hospital, Varanasi upto 12th March, 2012

Application form can be obtained by cash Rs 1500/- for General/OBC and Rs 1000/- for SC/ST candidates from the following branches of Bank of Baroda; Varanasi-BHU-SS Hospital branch, Ram Katora branch, Orderly bazaar branch; Delhi-Parliament street branch, Nehru place branch, Chowk Shakti nagar branch; Kolkata- College street branch; Lucknow-Charbagh branch; Hyderabad-Main branch, ABIDS circle; Jaipur-DCM branch; Bhopal-TT Nagar branch; Patna-Sri Krishnapuri; Bangalore-Main branch, Prithvi building, KG road; Pune-Darekar heights, 32, Karve nagar

By post from the office of the director (PMT-Cell), Institute of medical sciences, Banaras Hindu University, Varanasi-221005 by sending a written request through registered post along with crossed demand draft/bankers cheque (MICR only) of Rs 1600/- for general/OBC candidates and Rs 1100/- for SC/ST candidates of a nationalized bank, drawn in favour of The Director, Institute of Medical Sciences, BHU payable at Varanasi

Application form will be sent by speed/registered post

However, Institute will not be responsible for any loss or postal delay

The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (with candidate’s name and address on the back of demand draft)

Two self-addressed, preferably typed, white paper slips of 10 cm 6cm size must also accompany it

The requisition for PMT application form should be superscribed request for application form for PMT-2012, BHU

Last date of requisition for PMT form by post is 28th February, 2012

Incomplete/non-cashable draft (under amount/unsigned/without bank code number and seal) shall not be accepted

MO/IPO/Cheques shall not be accepted under any circumstances

Application forms from the office of the director (PMT-cell) institute of medical sciences, BHU, Varanasi-221005 can be obtained by post only and not in person up to 27th February, 2012

Last date for submission of completed application form: Application form duly completed must be sent through registered post so as to reach the director (PMT-Cell), Institute of medical sciences, Banaras Hindu University, Varanasi-221005 latest by 15th March, 2012

Application forms received after the last date shall not be accepted

Foreign nationals are not required to appear in the PMT examination for MBBS/BDS/BAMS courses, they may seek admission through the Ministry of External Affairs, Govt of India, New Delhi, as per the rules laid down for the purpose and their nomination must reach by 15th July, 2012
