Application details:

Students can apply for the UPCAT-Dental examination in the following two ways. Kindly choose only one option from below as per your own convenience:-

Option I – Pay through Demand Draft (DD) & Receive Application Form through Courier. (Cost Rs. 1100/-)

You will need the following for Ordering paper Application Form.

• Demand Draft (DD) of Rs. 1100/- in favour of “Association of Private Medical & Dental Colleges of U.P.” payable at Bareilly.

Steps for ordering paper Application Form

• Prepare a letter stating your name, address, phone & mobile.
• Arrange the Demand Draft (DD)
• Keep photocopy of DD for your own record.
• Send the letter and DD to the following address:-
  Oriental Bank of Commerce
  Springdell Branch, Bichpuri Road
  Pilibhit Bypass Road, 
  Bareilly –243006
  Uttar Pradesh, India

Note:- Association will not take any responsibility for timely delivery by Courier.

Option II – Collect paper Application Form from Authorized Centers after paying Cash. (Cost Rs. 1050/-)

Steps for collecting paper Application Form

• Approach Member Institutes or authorized Oriental Bank of Commerce branches to collect the Application Form directly.
