The Online Application Submission for Supplementary NEET UG 2013 for Urdu will end on 15th May 2013. The last date for receipt of confirmation page by CBSE is 18th May 2013.

The NEET UG 2013 Application Form has been released and can be filled by the candidates ONLINE only on the official website www.cbse.neet.nic.in. Prior filling the exam forms candidates should confirm their eligibility for exam as only those candidates will be considered for admissions who satisfy all the clauses of eligibility. Also, all the applicants should first read the form filling instructions and then fill the NEET UG 2013 Application Form.

The candidates have to take care of the following things, before filling the Application Form:

• Mode of Payment:

1. Online Payment: via Internet Banking, Credit Card or Debit Card using the online payment gateway.
2. Offline Payment: Directly deposit the NEET UG 2013 Application Form Fee in the CBSE Bank Account at the Syndicate Bank or Canara Bank or through NEFT at other banks by downloading the Bank Challan from the official website.
3. The candidates who want to deposit the NEET UG 2013 Application Form Fee via the online mode they have to keep their Credit / Debit Cards ready and must confirm their validity.
4. For the candidates who want to remit the fee via the bank challan or the offline mode, the candidates should complete the step 1 of the Registration process, download the bank challan, deposit it and complete the process of application.
5. The candidates have to fill the transaction id and other credentials of their NEET UG 2013 Application Form Fee Challan while logging onto the website to complete the application process.
6. The NEET UG 2013 Application Form Fee Submission Schedule is as follows:

1. The candidates have to bear charges other than the NEET UG 2013 Application Form fee i.e. processing or service charges. The details are available on the website.
2. There will be no refund of the NEET UG 2013 Application Form Fee and once submitted, the candidate can not withdraw the application.

Instructions for filling and sending the NEET UG 2013 Application Form / Confirmation Page:

• The candidates must confirm that they are eligible to appear in the exam before filling the application form. The candidates must go through the complete information brochure before filling the form.
• The Application Form has to be filled in ENGLISH language only.
• The candidates are advised to fill their complete and correct correspondence address, email address and contact numbers while filling the NEET UG 2013 Application Form as these credentials will be used for all the future correspondences.
• The correct code number for the qualifying exam has to be filled by the candidate, in case the exam code is not one of the 01 to 06, complete information should be furnished regarding the exam.
• The candidates who will be passing their class XII in 2013 have to fill the code as 07 in the NEET UG 2013 Application Form.

• The NEET UG 2013 Application Form should be sent via SPEED / REGISTERED POST only and it should be mailed before the last date to NEET UG 2013 Office.
• The NEET UG 2013 Application Form will not be accepted if submitted by any coaching institute or in person or courier or any other means than the Speed / Registered Post.
• All the correspondences in regard to the examination must be directed to the above mentioned address.

Procedure to send Confirmation Page:

• The candidates should take the print out of the Confirmation Page and the Board’s Address Slip which is to be pasted on a good quality A5 sized envelope. All the documents including the NEET UG 2013 Application Form should be sent in this envelope.
• The candidate must keep three copies of NEET UG 2013 Application Form or the confirmation page for future usage.
• The candidate must also preserve a copy of the fee remission proof for future usage.
• Last Dates for NEET UG 2013 Application Form Submission:

• The candidates should make sure that they fill their and their parents’ name and their date of birth correctly in the NEET UG 2013 Application Form as per the High School Certificate of Board / University.
• The J&K and Andhra Pradesh candidates have to submit an affidavit along with the NEET UG 2013 Application Form to avail seats under 15% All India Quota.

Photograph:

1. The candidates are advised to take 8 to 10 passport sized photographs with white background of dimensions 3.5” X 5”.
2. Before pasting the photograph on the confirmation page / NEET UG 2013 Application Form, the candidate must write down their names, contact number and application form number on its back side with ball point pen only.
3. The photographs must be taken on or after 1^st November, 2012 with candidate’s name and date indicated clearly below.
4. The candidates should not wear goggles or caps while taking the photograph.
