INSTRUCTIONS FOR COMPLETING & SENDING THE APPLICATION FORM AND OTHER DOCUMENTS GENERAL

1) A candidate seeking admission to the MBBS course at AFMC is required to send his/her application in the prescribed form (supplied separately with the prospectus). The application form can also be downloaded from website  www.afmc.nic.in. Such forms duly completed will also be acceptable along with demand draft for an amount of Rs 250/-(Rs two hundred and fifty only) drawn in favour of “Commandant, AFMC” payable to Pune. The Demand Draft must bear the Code Number of issuing Bank.

2) The candidate before filling up the Application Form shall satisfy his/her eligibility to appear in the examination.  The candidate is required to go through the prospectus carefully and acquaint himself/herself with all requirements with regard to the filling in of the application form and other documents.

3) Application form should be filled up in English and in block letters only. All squares, signature and addresses are to be written with Black pen and all circles to be darkened with Black pen only.

4) A sample of filled up application is attached at  Appendix ‘D’.  Candidates are advised to keep a photocopy of their completed application form for their record and future reference.

5) The application form is required to be sent under registered AD / speed post so as to reach the Officer in Charge Admissions, Dean Office, AFMC, Pune – 411 040 by due date.  Applications received after the last date of receipt will be rejected without intimation and no correspondence will be entertained in this regard. Receipt of Application form through couriers will not be accepted.

6) Application form must be sent in original and duly completed in all respects.  Photostat/Xerox copies/Fax copies will not be accepted.

7) Incomplete applications will be rejected without any intimation and no refunds or return of drafts will be asked for by candidates.  Request for any addition or alteration in the application form or any other document once submitted will not be entertained.

8) Last date for receipt of completed application form in AFMC is 05 Feb 2010.

9) The name must be entered in exactly the same fashion in the Application Form as mentioned in the mark sheet of qualifying examination.  Any variation is liable for rejection of the application.
